Abstract The medical record and health information practice laboratory have not used the disease index form, there is only an index of patients. The goal is to design a disease index form in Medical Record Laboratory STIKes Husada Borneo to support the completeness of learning in the Medical Record Practice Laboratory Stikes Husada Borneo Banjarbaru. This research method uses descriptive research with a qualitative approach. The subjects of this study were laboratory staff, student of medical record, and the head of the medical record and health information study program STIKes Husada Borneo Banjarbaru which amounted to 3 respondents.the index form in the Medical Record Laboratory STIKes Husada Borneo is designed Based on. Aspects that must be in the disease index this is form are physical aspects, anatomical aspects, and content aspects. In designing the disease index form to is in accordance with the needs by users of the form for the medical practice laboratory STIKes Husada Borneo Banjarbaru.
