Abstract Form design is the activity of designing forms based on the needs of service transactions or making service organizational reports.The contents of the medical record form are very complex because they contain demographic data and clinical data from patients entering to patients leaving or leaving health care facilities. Simpur Puskesmas only uses outpatient medical record forms. The purpose of this study was to design an outpatient medical record form at the Simpur Health Center. The research method used is descriptive. The subjects of this study were outpatient medical record forms and the research respondents were 3 medical record officers, doctors and nurses. Based on the results of the research, The outpatient medical record form at the Simpur Health Center already has an outpatient medical record form but it is still incomplete in the patient identity section.
