Abstract
Medical record is a file that contains notes about the patient's identity, examination, treatment, action, and other services that have been provided to the patient. Medical record documents (DRM) must be maintained as well as possible because the medical record file for the puskesmas is a source of memory and as a source of information that must be accounted for as well as possible. Based on a preliminary study conducted at the Landasan Ulin Health Center, problems were found, namely once found rats in the filling room, no camphor and thermometer. The purpose of this study was to determine the factors causing damage to medical record files at the Landasan Ulin Health Center. This research method is a descriptive method with a cross sectional approach. This research instrument uses observation guidelines and interview guidelines. The results of this study found that the physical factor causing the damage to the medical record file was that there were no curtains/curtains on the window of the room. Meanwhile, sunlight is good because it doesn't hit the beam surface directly, and the average normal room temperature is 24.8oC. Biological factors that cause damage to medical record files are that there is no camphor on the storage shelf. The filling room is safe from pests (cockroaches, moths, termites and rats) and there is no file damage caused by pests and is safe from fungi. Chemical factors that caused file damage included some files not using black ink and faded, the files being stained with food or drink, and there were officers eating and drinking in the filling room. Where the results show that the file uses black ink and does not fade easily as many as 78 (78%) medical record files, and there are no food or drink stains as many as 94 (94%) medical record files.
